Clinical Medicine Domain Expert | $70–110/hr | Hybrid – Bay Area, CA

Join a leading AI lab's GenAI research team as a senior Clinical Medicine Domain Expert. In this hybrid, full-time role, you will apply your deep clinical knowledge to shape how frontier AI models reason about real-world medical practice — reviewing outputs, writing instruction specs, and building benchmarks that define what clinical excellence looks like for AI systems.

This is a W-2 employment position with Cincinnatus LLC, placing you directly within a top AI lab's extended workforce, working alongside their research teams with client-issued tools and equipment.

Key Responsibilities

  • Data QA & Review: Evaluate clinical knowledge tasks and model outputs, identifying unsafe recommendations, thin reasoning, off-guideline answers, and outputs that appear correct but would not withstand clinical scrutiny.
  • Instruction Specs & Golden Datasets: Author high-quality instruction specifications, produce golden solutions to clinical problems, and define new tasks that reflect how medicine is genuinely practiced.
  • Benchmarks & Evaluation: Design challenging clinical evaluation sets and help build medicine-specific skills and tools in collaboration with the research team.
  • Calibration: Partner with researchers and adjacent specialists to maintain consistent standards, translating tacit clinical judgment into explicit, teachable criteria.

Core Qualifications

  • Education: MD or DO from an accredited medical school with a completed residency in a recognized specialty.
  • Experience: 4+ years of post-residency clinical practice (residency/fellowship alone does not qualify).
  • Licensure: Active, unrestricted U.S. medical license and board certification in your specialty.
  • Domain Depth: Genuine specialization in a defined clinical area such as internal medicine, oncology, radiology, emergency medicine, surgery, psychiatry, or a medical subspecialty. Experience in utilization management, clinical informatics, or medical affairs is a plus.
  • Seniority: Demonstrated progression to a senior role — Attending Physician, Medical Director, Division Chief, Associate/Full Professor, or CMO — with real ownership of clinical decisions.
  • AI Fluency: Hands-on experience using large language models professionally, with the judgment to distinguish well-reasoned answers from plausible-sounding errors.
  • Availability: Able to commit to 40 hours per week for an initial 6-month engagement.
  • Location: Must reside in the Bay Area, CA, or be willing to relocate at your own cost prior to the engagement start. Relocation assistance is not provided.
  • Excellent written communication skills with the ability to deliver precise, well-structured feedback.
